A range of information is available to health professionals who have a client or patient who they think is at risk of sustaining a fall, or who have previously fallen. This information includes an assessment/ screening and referral care pathway which outlines the steps that health professionals should go through when assessing a client’s risk of falling. This also details the referral forms that are required for given areas within South Australia where available. You can give your clients brochures detailing how they can help reduce their falls risk. Some of this information includes vision, exercise and balance, medications, incontinence, foot care and safe shoes. There is information on professional development opportunities including training courses, conferences and seminars, as well as key research that has been completed in falls prevention or related areas. Details on falls prevention policies and guidelines are listed, with links directly to these documents. In the SA programs and activities section there is information on the falls networks within South Australia, a listing of falls prevention programs, and directories of falls prevention programs that can be searched for services within a given area. There is also a link to the Falls SA discussion forum where questions can be posted for comment or feedback, and discussion of topics made.
